L'atteinte vasculaire varie de structure selon la forme d'hypertension artérielle. Dans l'hypertension artérielle chronique dite bénigne, l'artériosclérose est quasi constante. Dans l'hypertension artérielle dite maligne, les lésions vasculaires constituent l'élément le plus caractéristique du tableau anatomique.
